COMPLIMENTS AND CONCERNS PRIVACY STATEMENT AND TERMS OF USE
This Privacy Statement applies to the electronic Compliment and Concern Form
for compliments, concerns, complaints and recommendations portion of the Thunder
Bay Regional Health Sciences Centre (TBRHSC) website.
TBRHSC is committed to protecting the privacy of visitors to our website. This Privacy
Statement explains how personal information may be collected, used or disclosed
through the Compliment and Concern process. Please read
it carefully. This document was last updated April 21, 2022.
Please note that our electronic Compliment and Concern Form is not continuously monitored.
The submission must not be used for time-sensitive matters, including for the purpose
of seeking health care or requesting/scheduling an appointment at TBRHSC.

Collection of Personal Information
In order for TBRHSC to properly review and address the Compliments and Concerns submitted to us,
it is necessary to collect personal information.
“Personal information” is information that is personally identifiable, such as your
name, address, email address, telephone number, personal health information, and
the compliment or concern that you provide to us. “Personal health information” includes, but
is not limited to, information relating to an individual’s health and health care
including the fact that the individual received care at TBRHSC, the nature of the
care provided and the name of his/her health care professional(s).
The personal information requested on our Compliment and Concern Form
is collected in accordance with the Freedom of Information and Protection of Privacy
Act (FIPPA) and the Personal Health Information Protection Act (PHIPA), as applicable.
The information will not be used for any purposes other than those stated in this
Privacy Statement, unless we have received consent or are permitted or required by
law.
You should only provide us with personal information belonging to someone else if
you have that person’s consent, or are his/her legal decision-maker. If you have
included someone else’s personal information in your Compliment and Concern
Form, we will assume for the purpose of collecting this information
that you have proper authority to share it with us. We reserve the right to make
inquiries and to verify that authority before proceeding with our process.
Use of Personal Information
TBRHSC uses the personal information you provide in the Compliment and
Concern Form to review and respond to your compliment or concern. This may
require us to share your personal information internally with the individuals involved
in providing the care and/or services that relate to your compliment or concern, and with staff,
management, senior administration and/or the Board of Directors in order to address
concerns, implement changes and/or make improvements. Your personal information
will also be used to contact you regarding your compliment or concern, including:
- to confirm receipt of your compliment or concern;
- to explain our processes for reviewing and addressing your compliment or concern;
- to follow up on your compliment or concern, including to obtain additional information from you
if needed; and/or
- to provide you with a response.
We cannot respond to a compliment or concern submitted anonymously.
If you provide us with your email address, TBRHSC may contact you by email for these
purposes. Please note that sending email over the internet is not secure, due to
the potential for it to be misdirected, intercepted and/or manipulated and re-transmitted.
Disclosure of Personal Information
TBRHSC will not disclose the personal information you provide except with consent
or as otherwise permitted or required by law. “Required by law” refers to situations
where TBRHSC is required by court order or other legal or regulatory process, including
a mandatory reporting obligation, to disclose personal information to law enforcement
authorities, the courts, regulatory authorities, or another third party.
Retention
In accordance with TBRHSC’s record retention practices, personal information provided
through Compliments and Concerns Forms is only kept until
it is no longer necessary or relevant for the identified purposes, subject to retention
periods or legal requirements. All personal information destroyed or erased from
our records is done in a secure manner.
Server/Web Logs
TBRHSC maintains server logs and web logs which automatically collect and log all
incoming traffic to our website including your Internet Protocol (IP) address. However,
we make no attempt to link these addresses with the identity of individuals visiting
our site.
Cookies
Like most websites, our website may set and access “cookies” on your computer. Cookies
are text files we place in your computer’s browser to store your preferences. We
may use cookies to understand website usage and to improve the website’s content
and offerings. For example, we may use cookies to personalize your experience (e.g.
to recognize you as a member of the public when you return to our site or to determine
the appropriate font size for the page). Cookies on our website do not contain any
personal information and are only used to facilitate the operation of the TBRHSC
website.
Security
Information collected and transmitted via this Compliment and
Concern Form is SSL (secure socket
layer) encrypted, a standard security technology for establishing an encrypted link
between the web server and the browser using 128-bit encryption. TBRHSC makes every
attempt to secure the integrity of the data being collected through routine patching
and maintenance of servers and network equipment. However, sophisticated types of
attacks can defeat the security provided by SSL and therefore information submitted
may be subject to interception, modification, and/or retransmission.
